A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS, FLORIDA, ACCEPTING A FIVE HUNDRED
ELEVEN THOUSAND ONE HUNDRED AND FORTY-SIX
DOLLAR AND TWENTY-FIVE CENTS ($511,146.25) GRANT
AWARD FROM THE DEPARTMENT OF HEALTH AND HUMAN
SERVICES, CENTERS FOR DISEASE CONTROL AND
PREVENTION (CDC) FOR RACIAL AND ETHNIC APPROACHES
TO COMMUNITY HEALTH (REACH), NUNC PRO TUNC TO
APRIL 1, 2021; AUTHORIZING THE CITY MANAGER TO
EXECUTE ANY AND ALL DOCUMENTS RELATED TO THE
ACCEPTANCE OF THIS AWARD; PROVIDING FOR THE
ADOPTION OF REPRESENTATIONS; PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the City of Miami Gardens through Live Healthy Miami Gardens
(LHMG) has been awarded a grant by the Department of Human Services, Centers for
Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community
Health (REACH) to plan and execute a comprehensive campaign to support vaccination
coverage for racial and ethnic groups in the City of Miami Gardens experiencing
disparities in vaccination rates, and
WHEREAS, the 18-month grant (April 2021-September 2022) will leverage
LHMG’s current REACH project and Flu Vaccination work to maximize efforts and
community reach, and
WHEREAS, components of the project includes a comprehensive communications
campaign, education and training, community outreach, and administered vaccinations,
and
WHEREAS, the project will be implemented by the LHMG Initiative, which was
started in 2014 to drive citywide changes that measurably improve the health and life
outcomes of people who live and work in the City of Miami Gardens, and
WHEREAS, the total grant award of Five Hundred Eleven Thousand One Hundred
and Forty-Six Dollars and Twenty-Five Cents ($511,146.25) will be used to fund the
project in its entirety, and
WHEREAS, Staff recommends that the City Council approve this Resolution Nunc
Pro Tunc to April 1, 2021, authorizing the City Manager to execute any and all documents
related to the acceptance of this award,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:

Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.
Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby accepts a Five Hundred Eleven Thousand One Hundred and Forty-Six Dollar and
Twenty-Five Cents ($511,146.25) grant award from the Department of Health and Human
Services, Centers for Disease Control and Prevention (CDC) for Racial and Ethnic
Approaches to Community Health (REACH), Nunc Pro Tunc to April 1, 2021, and
authorizes the City Manager to execute any and all documents related to the acceptance
of this award.
Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon
its final passage.

GARDENS AT ITS REGULAR MEETING HELD ON JUNE 23, 2021.
