HomeMy WebLinkAbout2020-105-3503 Grant Award from CDC REACH RESOLUTION NO. 2020-105-3503 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA, ACCEPTING A SIX HUNDRED NINETY-TWO THOUSAND FIVE HUNDRED AND EIGHTY- EIGHT DOLLARS ($692,588.00) YEAR THREE (3) GRANT AWARD, FROM THE CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC), RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH (REACH) GRANT; AUTHORIZING THE CITY MANAGER TO EXECUTE ANY AND ALL DOCUMENTS RELATED TO THE ACCEPTANCE OF THIS AWARD; FURTHER AUTHORIZING THE CITY MANAGER TO RENEW EXISTING CONTRACTOR AGREEMENTS AND ALLOCATING THIRD YEAR GRANT FUNDING; AUTHORIZING THE CITY MANAGER AND THE CITY CLERK TO EXECUTE AND ATTEST RESPECTIVELY CONTRACTOR AGREEMENTS FOR THIS PURPOSE IN SUBSTANTIAL FORM AS ATTACHED HERETO AS EXHIBIT A; PROVIDING FOR INSTRUCTIONS TO THE CITY CLERK; PROVIDING FOR THE ADOPTION OF REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, in an effort to support the work of Live Healthy Miami Gardens (LHMG), City of Miami Gardens ("City") Staff submitted a multi-year grant to the Department of Human Services, Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community Health (REACH) (CDC-RFA-DP18-1813), and WHEREAS, the City of Miami Gardens was awarded Six Hundred Ninety-Two Thousand Five Hundred and Eighty-Eight Dollars ($692,588.00) from the CDC REACH project annually for the period of 2018 through 2022, and WHEREAS, the 5-year CDC program is designed to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease, and WHEREAS, as a requirement of the CDC's Notice of Funding Availability (NOFA), the initiative is required to use a Collective Impact approach and partner with multiple organizations who work collaboratively to implement the funded strategies, and WHEREAS, Contractor Agreements between the City and service providers are required to continue implementing CDC strategies for our CDC Year 3 grant, and WHEREAS, the attached agreements reflect the ten organizations the City desires to continue contracting with in Year Three (3) commencing on September 29, 2020: 1. Alliance for a Healthier Generation - $65,000.00 2. Change Lab Solutions - $9,500.00 3. Common Threads - $5,399.40 4. Flipany - $10,719.03 5. Florida Impact to End Hunger - $68,601.47 6. Jessie Trice Community Health System - $60,880.00 7. Trinity Church/Peacemakers Family Center - $67,514.00 8. QQ Consultants - $70,045.00 9. Urban Health Partnerships - $26,805.00 10. Urban Oasis Project - $13,221.50 WHEREAS, Staff recommends that the City Council accept the grant award for Year Three (3) from the CDC REACH and authorize the City Manager to execute contractor agreements and all documents related to this award, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA AS FOLLOWS: Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas paragraphs are hereby ratified and confirmed as being true, and the same are hereby made a specific part of this Resolution. Section 2: ACCEPTANCE OF GRANT/AUTHORIZATION: The City Council of the City of Miami Gardens hereby accepts the Six Hundred Ninety-Two Thousand Five Hundred and Eighty-Eight Dollars ($692,588.00) award, from the CDC REACH grant and authorizes the City Manager to execute any and all documents related to the acceptance of this award. The City Council of the City of Miami Gardens further authorizes the City Manager to renew existing contractor agreements, authorizes the City Manager and the City Clerk to execute and attest respectively contractor agreements in substantial form as attached hereto as Exhibit A and allocates the Third Year Grant funding as follows: 1. Alliance for a Healthier Generation - $65,000.00 2. Change Lab Solutions - $9,500.00 3. Common Threads - $5,399.40 4. Flipany - $10,719.03 5. Florida Impact to End Hunger - $68,601.47 6. Jessie Trice Community Health System - $60,880.00 7. Trinity Church/Peacemakers Family Center - $67,514.00 8. QQ Consultants - $70,045.00 9. Urban Health Partnerships - $26,805.00 10. Urban Oasis Project - $13,221.50 Section 3: INSTRUCTIONS TO THE CITY CLERK: The City Clerk is hereby authorized to obtain two (2) fully executed copies of the subject Agreements with one to be maintained by the City, and one to be delivered to each organization. Page 2 of 3 Resolution No. 2020-105-3503 Section 4: EFFECTIVE DATE: This Resolution shall take effect immediately upon its final passage.
