HomeMy WebLinkAbout2022-159-3870_-_CDC_Grant_Agreement_-_Adopted_-_PdfRESOLUTION NO. 2022-159-3870 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA, ACCEPTING A NINE HUNDRED AND FOUR THOUSAND SEVEN HUNDRED AND NINETY ONE DOLLARS ($904,791.00) GRANT AWARD FROM THE SERVICES,HUMAN ANDHEALTHOFDEPARTMENT CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) FOR RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH (REACH), NUNC PRO TUNC TO OCTOBER 1, 2022; AUTHORIZING THE CITY MANAGER TO EXECUTE ANY AND ALL DOCUMENTS RELATED TO THE ACCEPTANCE OF THIS AWARD;PROVIDING FOR THE ADOPTION OF REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, the City of Miami Gardens through Live Healthy Miami Gardens (LHMG) has been awarded a grant by the Department of Human Services, Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community Health (REACH) to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease, and WHEREAS, this grant is year five of a five year grant award with a start date of October 1, 2022 and end date of September 29, 2023, and WHEREAS, the goal of the CDC REACH project is to reduce chronic disease cases and deaths in Miami Gardens by increasing resident access of preventive and primary care resources, increasing the supply of and demand for healthy local foods, and transforming built and social environments to sustain improved physical activity levels and nutrition quality, and WHEREAS, the total grant award is Nine Hundred and Four Thousand Seven Hundred and Ninety One Dollars ($904,791.00), and WHEREAS, Four Hundred Ninety-Nine Thousand Four Hundred and Ninety One Dollars ($499,491.00) will be allocated to Reach Rise and Four Hundred and Five Thousand and Three Hundred Dollars ($405,300.00) will be allocated to CDC Covid-19 Grant projects, and WHEREAS, Staff recommends that the City Council approve this Resolution Nunc Pro Tunc to October 1, 2022, authorizing the City Manager to execute any and all documents related to the acceptance of this award, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA AS FOLLOWS: Resolution No. 2022-159-3870 DocuSign Envelope ID: 3CD462A9-79D2-48F9-95A0-E8E081E239EB Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas paragraphs are hereby ratified and confirmed as being true, and the same are hereby made a specific part of this Resolution. Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens hereby accepts a Nine Hundred and Four Thousand Seven Hundred and Ninety One Dollar ($904,791.00) grant award from the Department of Health and Human Services, Centers for Disease Control and Prevention (CDC) for Racial and Ethnic Approaches to Community Health (REACH), Nunc Pro Tunc to October 1, 2022, and authorizes the City Manager to execute any and all documents related to the acceptance of this award. Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon its final passage.
