A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS,FLORIDA, AUTHORIZING THE CITY
MANAGER TO RENEW THAT CERTAIN GUARDIAN VISION
AND DENTAL PLAN AGREEMENT UNDERWRITTEN BY THE
GUARDIAN LIFE INSURANCE COMPANY OF AMERICA
AGREEMENT, ON COMMENCING JAN AND20231, UARY
ENDING ON DECEMBER 31, 2023; PROVIDING FOR THE
ADOPTION OF REPRESENTATIONS;PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, on March 18, 2020, staff issued Invitation to Bid number 19-20-016,
Agent of Record Insurance Broker via BidSync, and
WHEREAS, after evaluation, scoring and oral presentation, the City Council
awarded the bid to Acentria Public Risk to act as the Agent of Record Insurance Broker
for the City of Miami Gardens, and
WHEREAS, Acentria Public Risk presented and at the City of Miami Gardens
selected the Guardian Life Insurance Company of America (“Guardian”) to provide dental
and vision coverage for City of Miami Gardens (“City”) employees at the employees’ cost,
and
WHEREAS, the contract with Guardian for dental and vision coverage expires on
December 31, 2022, and
WHEREAS, Acentria Public Risk has proposed the option to renew the contract
for dental and vision coverage with Guardian for one year commencing on January 1,
2023, and ending on December 31, 2023, and
WHEREAS, the dental Preferred Provider (“PPO”) Coverage has an increase of
7.16% due to utilization and the vision plan has a 10.04% increase due to high utilization,
and
WHEREAS, the rates for the Managed Dental Care (“DHMO”) has not changed,
and
WHEREAS, City Staff recommends that the City Council authorize the City
Manager to execute a one (1) year renewal for dental and vision coverage with Guardian,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:

Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.
Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby authorizes the City Manager to renew that certain Guardian Vision and Dental
Plan Agreement underwritten by the Guardian Life Insurance Company of America,
commencing on January 1, 2023, and ending on December 31, 2023.
Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon
its final passage.

GARDENS AT ITS REGULAR MEETING HELD ON SEPTEMBER 28, 2022.
