A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS,FLORIDA, AUTHORIZING THE CITY
MANAGER AND THE CITY CLERK TO EXECUTE AND ATTEST
SUBRECIPIENT AGREEMENTS WITH LIVE HEALTHY MIAMI
GARDENS THEPARTNERS FROM (LVMG) COMMUNITY
GRANT AWARDED TO THE CITY BY THE CENTERS FOR
DISEASE CONTROL AND PREVENTION (CDC) FOR RACIAL
AND ETHNIC APPROACHES TO COMMUNITY HEALTH
OF CITYTHEREVIEWTHETO SUBJECT(REACH);
ATTORNEY; PROVIDING FOR INSTRUCTIONS TO THE CITY
CLERK;PROVIDING FOR THE ADOPTION OF
REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, in October of 2022, the CDC issued a Notice of Award to the City of
Miami Gardens to continue its REACH and Vaccination Projects for fiscal year September
20, 2022 - September 29, 2023 in the amount of Nine hundred Thousand Four Thousand
Seven hundred and Nnty-One Dollars ($904,791.00), and
WHEREAS, the item to accept the CDC Grant was passed by our City Council on
October 23, 2022, Nunc Pro Tunc to September 30, 2022, and
WHEREAS, LHMG is executing agreements with organizations who will be funded
to continue implementing REACH and Covid/Flu projects, and
WHEREAS, the agreements are with the following organizations for the REACH
projects: 1) Alliance for a Healthier Generation for $46,348.00; 2) Common Threads for
$7,649.40; 3) Florida Impact for $68,601.47; 4) Jessie Trice Community Healthy System
(JTCHS) for $61,240.00; 5) QQ Research Consultants for $73,045.00; 6) Trinity Church
–Community Clinical Linkages for $69,510.00; 7) Trinity Church – Faith and Wellness for
$57,586.88; 8) Urban Health Partnerships for $119,970.00; 9) Yemaya Community
Solutions for $16,463.00, , and
WHEREAS, the agreements are with the following organizations for the
Supplemental Vaccine Project: 1) Abundant Living Health and Wellness for $19.500.00;
2) Center for Family and Child Enrichment for $66,198.00; 3) Jessie Trice Community
Healthy System for $74,512.00; 4) MD Marketing Network for $36,750.00; 5) QQ
Research Consultants for $33,000.00; 6) UHI Community Care Clinic for $32,182.00; 7)
UMC Free Clinic for $32,182.00.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:
Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.

Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby authorizes the City Manager and the City Clerk to execute and attest subrecipient
agreements with LVMG Community Partners: Alliance for a Healthier Generation;
Common Threads; Florida Impact; Jessie Trice Community Healthy System (JTCHS);
QQ Research Consultants; Trinity Church – Community Clinical Linkages; Trinity Church
– Faith and Wellness; Urban Health Partnerships; Yemaya Community Solutions;
Abundant Living Health and Wellness; Center for Family and Child Enrichment; Jessie
Trice Community Healthy System; MD Marketing Network; QQ Research Consultants;
UHI Community Care Clinic; UMC Free Clinic and subject to the review of City Attorney.
Section 3: INSTRUCTIONS TO THE CITY CLERK: The City Clerk is hereby
authorized to obtain two (2) fully executed copies of the subject Agreements with one to
be maintained by the City, and one to be delivered to each Subrecipient.
Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon
its final passage.

GARDENS AT ITS REGULAR MEETING HELD ON DECEMBER 14, 2022.
