A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS, FLORIDA, AUTHORIZING THE CITY
MANAGER AND CITY TO EXECUTE AND ATTEST,
RESPECTIVELY, THAT CERTAIN HEALTH EQUITY GRANT
AGREEMENT, IN THE AMOUNT OF FOUR HUNDRED NINETY-
EIGHT THOUSAND SIX HUNDRED TWENTY-SEVEN DOLLARS
AND SIXTY-THREE CENTS ($498,627.63), WITH THE FLORIDA
DEPARTMENT OF HEALTH; PROVIDING FOR THE ADOPTION
OF REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE
DATE.
WHEREAS, in May 2023 the City of Miami Gardens’ Live Healthy Miami Gardens
(LHMG) was awarded a grant from the Florida Department of Health (FDOH) under their
National Initiative to Address COVID-19 and Health Disparities in the amount of Four
Hundred Ninety-Eight Thousand Six Hundred Twenty-Seven Dollars and Sixty-Three
Cents ($498,627.63), and
WHEREAS, the proposed term of the Health Equity Agreement (Agreement) is
from May 1, 2023, to March 31, 2024, and will implement programs that meaningfully and
measurably close health disparity gaps and foster health equity throughout the City of
Miami Gardens, and
WHEREAS, the proposed objects of the Agreement are to (1) improve healthcare
access and quality, (2) improve social community context, and (3) improve neighborhood
and built environment, and
WHEREAS, all proposed project cost will be covered by the grant in the Fiscal
Year 2024 (FY24) Budget, and
WHEREAS, City Staff recommends that the City Council authorize the City
Manager to execute that certain Health Equity Grant Agreement, in the amount of Four
Hundred Ninety-Eight Thousand Six Hundred Twenty-Seven Dollars and Sixty-Three
Cents ($498,627.63), with the Florida Department of Health,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:
Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.
Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby authorizes the City Manager and City to execute and attest, respectively, that
certain Health Equity Grant Agreement, in the amount of Four Hundred Ninety-Eight

Thousand Six Hundred Twenty-Seven Dollars and Sixty-Three Cents ($498,627.63), with
the Florida Department of Health.
Section 3: INSTRUCTIONS TO THE CITY CLERK: The City Clerk is hereby
authorized to obtain two (2) fully executed copies of the subject Agreement with one to
be maintained by the City, and one to be delivered to the Florida Department of Health.
Section 4: EFFECTIVE DATE: This Resolution shall take effect immediately upon
its final passage.
