OF THE CITY OF MIAMI GARDENS, FLORIDA, AUTHORIZING THE ISSUANCE OF A CHANGE ORDER TO TO J & S TIRE AND AUTO, INC. D/B/A FLEET TIRE TRUCK AND AUTO CENTER, INC. IN AN AMOUNT NOT TO EXCEED
FORTY THOUSAND DOLLARS ($40,000.00), TO THE EXISTING AGREEMENT FOR FLEET MAINTENANCE AND REMEDIAL REPAIRS; PROVIDING FOR THE ADOPTION OF REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE DATE.
Staff Summary: Background In an effort to provide preventative maintenance and remedial repairs to the City’s fleet vehicles, Council approved the award for Fleet Vehicle Preventative
Maintenance Services to Fleet Tire Truck and Auto Center, located Hollywood, FL on June 24, 2009. We are currently in the final year of a three (3) year contract. A replacement solicitation
is being prepared and is expected to be finalized before the present contract’s expiration of June 23, 2012. ITEM K-3) CONSENT AGENDA RESOLUTION Change Order to J&S Tire and Auto Inc.
1515 NW 167 Street, Building 5 Suite 200 Miami Gardens, Florida 33169 Current Situation Expenditures for repairs at Fleet Tire Truck and Auto Center till the expiration date are anticipated
at $80,000. Because this exceeds the City Managers authority, council approval is requested.

an amount not to exceed the allocated budget through contract expiration date of 6/23/2012. Attachment:
