Staff Summary: Background: The Miami Gardens City Hall Municipal Complex project is being developed with a phased approach to expedite the design and construction. Phase 2 of the project consists of the actual construction of the City Hall building, Police Department building, Mechanical building, Parking Garage and the Off ‐Site Roadway Improvements. 1515 NW 167 Street, Building 5 Suite 200 Miami Gardens, Florida 33169 developed into Guaranteed Maximum Price #2 (GMP#2) by Skanska USA Building. The City reached an agreement with Skanska USA Building and the Notice of Award was issued on June 4, 2012. Current Situation: Council awarded a contract to GFA International Inc. for continued services under ITB# 08 ‐09 ‐047 entitled “Inspection and Testing Laboratory Services” to provide inspection and testing services for city projects. The testing services that GFA International, Inc., would provide for the CMG Municipal Complex are for Vibrio Compaction, Roofing, Fire Proofing, and Engineering Services. GFA International, Inc. provided the lowest quotations for several categories for Testing and Laboratory services. ITB#08 ‐09 ‐047 will expire August 28, 2012; however, GFA International, Inc. has agreed to maintain the quoted prices and the same terms and conditions throughout the duration of the project. This will ensure the integrity of the inspection and testing services. The attached quotation associated with GMP #2 was evaluated for compliance with the contract documents and pricing provided under ITB# 08 ‐09 ‐047. Proposed Action: It is recommended that the City Council approve the attached resolution authorizing the City Manager to issue a purchase order to GFA International, Inc., located in Coral Gables, Florida, in an amount not to exceed $73,562.78 to provide Inspection and Testing Lab services for Vibro Compaction, Roofing, Fire Proofing, and Engineering Services for the construction of the new CMG Municipal Complex. CATEGORY - 4.VIBRO COMPACTION GFA’s proposed scope of services will include pr oviding an experienced technician at the site to oversee critical aspects of the earthwork su ch as the vibro flotatio n, and documenting site earthwork progress. In addition to the technician , a Staff Engineer will make periodic visits and a Professional Engineer will certify all reports. We wil l work with the site contractor to coordinate the optimal timing. We ask that critical aspects of the site development only occur with the GFA technician present. The above unit rates and/or hourly rates do not include any overtime work. Overtime work (scheduled for before 7:00 AM or going past 4:00 PM Monday through Friday, more than 8 hours in one day, more than 40 hours in one week, or anytime on Satur days, Sundays, or Holi days), will be charged at the standard rate times 1.5. Technician Time will be charged in the ev ent that the Contractor’s work is not ready for testing/inspection upon arrival of GFA’s representative, or if our minimum testing frequencies specified herein cannot be achieved. A typed report, signed and sealed by a State of Flor ida Registered Professi onal Engineer will be provided. In accordance with Florida Statues C hapter 471 and the rules of the Florida Board of Professional Engineers (Chapter 61G 15), a Professional Engineer is required to perform a thorough review of each report before affixi ng his seal and signing for certification for each report’s validity Please note that GFA’s services do not include supervis ion or direction of the actual work. Also, be aware that neither t he presence of our field repr esentative nor the observation and testing by our firm shall imply GFA’s responsibility for defects discovered in the construction work. It is understood that GFA will not be responsible for job or site safety on this project. Job site safety will be the sole responsibility of the contractor. 2. CATEGORY - 10.ROOFING GFA will perform Roofing Inspections with a BPR-cer tified inspector and/or a RCI-certified Registered Roof Observer periodically during the roof system installation. The in spector to issue written report(s) signed and sealed by a State of Flor ida Registered Professi onal Engineer. Services to be performed on City Hall, Police Building and Mechanical Building. GFA will provide inspectors that are licensed and certified under Fl orida Statutes Chapters, 468, 471 or 481. The services GFA proposes to provide will include t he required inspections for the refe renced project, as outlined within Section 109 of the Florida Buil ding Code, Florida Statute 468 r egarding building inspection and requirements FBC 2122.1, FBC 1 04.6.4.1 & FBC 2510.6.4.5. GROSS CONTINGENCY FEE –ADDITIONAL TESTING GFA understands that additional testing may be requir ed for the Future City Hall Complex. In anticipation of any additional testing or inspecti ons not covered in the abov e quote, GFA will provide a contingency quote of 10%. All rates for testing and inspections under the contingency section shall be billed at the same rates as quoted in the abov e proposal. Please note any additional testing or inspections surpassing the 10% contingency fee sh all be billed separately to the City of Miami Gardens. CONDITIONS AND QUALIFICATIONS These services include an engineering technician or inspector, field sampling, travel, test equipment and a typed report signed and sealed by a State of Florida Regist ered Professional Engineer. The rates for professional engineering services apply to report review, site visits and meetings, consultations, engineering letters, and le tters of intent and certification. Please note that the overall cost of constructi on quality control program s is dependent upon the contractor’s scheduling, weather and other factors beyond our contro l. However, you will only be invoiced for services rendered. Hourly rates will be charged in whole hour increments with fractions of an hour rounded up to the next full hour. All hourly labor rates are portal to portal. GFA will provide detailed invoices on completed work . Our terms are net 30 days. All GFA services must be paid in full, including final letter, prior to delivery of any Final Letters of Compliance. The pricing contained herein is in accor dance with. Additional items not lis ted in this proposal will be quoted upon request. Reports wil l be distributed promptly to a maximu m of five (5) interested parties without charge (Ex: Building Department , Owner, and Contractor). Additi onal reports will be billed at 1% of the monthly invoice for each additional party.
