A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS, FLORIDA, AUTHORIZING THE CITY
MANAGER AND CITY ATTORNEY TO NEGOTIATE AND
EXECUTE A TWO (2) YEAR AGREEMENT WITH GUARDIAN
DENTAL PLANS UNDERWRITTEN BY THE GUARDIAN LIFE
INSURANCE COMPANY OF AMERICA, COMMENCING ON
JANUARY 1, 2020 AND ENDING ON DECEMBER 31, 2021;
PROVIDING FOR ADOPTION OF REPRESENTATIONS; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, Humana, Inc.'s ("Humana") annual contract for dental coverage
expires on December 31 , 2019, and
WHEREAS, the City of Miami Gardens ("City") hired an agent of
record/insurance broker (PRIA) to solicit proposals from insurance carriers, including
the existing provider, and
WHEREAS, PRIA obtained three proposals for dentals plans from Humana,
Delta Dental Plans Association, a not-for-profit organization, and the Guardian Dental
Plans underwritten by the Guardian Life Insurance Company of America., and
WHEREAS, for Preferred Provider (PPO) Coverage and for Managed Dental
Care (DHMO) Coverage, Humana's proposed two (2) year guaranteed annual
estimated premium was $181,262, Guardian Dental Plans' proposed two (2) year
guaranteed annual estimated premium was $174,726 and Delta Dental Plans
Association's proposal was not considered because it did not offer vision coverage, and
WHEREAS, because of additional orthodontia, implant and lab fee coverage,
City Staff recommends that the City enter into a two (2) year agreement with Guardian
Dental Plans,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:
Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.
Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby authorizes the City Manager and City Attorney to negotiate and execute a two
(2) year agreement with Guardian Dental plans underwritten by the Guardian Life
Insurance Company of America, commencing on January 1, 2020 and ending on
December 31 , 2021.
Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately
upon its final passage.
PASSED AND ADOPTED BY THE CITY COUNCIL OF THE CITY OF MIAMI
GARDENS AT ITS REGULAR MEETING HELD ON SEPTEMBER 25, 2019.
