A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
MIAMI GARDENS, FLORIDA, AUTHORIZING THE CITY
MANAGER AND CITY ATTORNEY TO NEGOTIATE AND
EXECUTE A TWO (2) YEAR AGREEMENT WITH GUARDIAN
VISION PLAN UNDERWRITTEN BY THE GUARDIAN LIFE
INSURANCE COMPANY OF AMERICA, COMMENCING ON
JANUARY 1, 2020 AND ENDING ON DECEMBER 31 , 2021;
PROVIDING FOR ADOPTION OF REPRESENTATIONS; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, Humana, Inc.'s ("Humana") annual contract for vision coverage
expires on December 31, 2019, and
WHEREAS, the City of Miami Gardens ("City") hired an agent of
record/insurance broker (PRIA) to solicit proposals from insurance carriers, including
the existing provider, and
WHEREAS, PRIA obtained three proposals for group vision coverage from
Humana, Aetna Better Health of Florida, and the Guardian Vision Plan underwritten by
the Guardian Life Insurance Company of America, and
WHEREAS, Humana's estimated annual premium cost was $38,669, Guardian
Vision Plan's estimated annual premium cost was also $38,669 and Aetna Better Health
of Florida's estimated annual premium cost was 36% higher than the current plan cost,
and
WHEREAS, because of an enhanced provider network, an increased contact
lens allowance and increased non-network reimbursements, City Staff recommends that
the City enter into a two (2) year agreement with Guardian Vision Plan,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF MIAMI GARDENS, FLORIDA AS FOLLOWS:
Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas
paragraphs are hereby ratified and confirmed as being true, and the same are hereby
made a specific part of this Resolution.
Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens
hereby authorizes the City Manager and City Attorney to negotiate and execute a two
(2) year agreement with Guardian Vision Plan underwritten by the Guardian Life
Insurance Company of America, commencing on January 1, 2020 and ending on
December 31 , 2021.
Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately
upon its final passage.

GARDENS AT ITS REGULAR MEETING HELD ON SEPTEMBER 25, 2019.
