HomeMy WebLinkAbout2019-264-3393 CDC Grant Acceptance RESOLUTION NO. 2019-264-3393 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA, ACCEPTING A SIX HUNDRED NINETY-TWO THOUSAND FIVE HUNDRED AND EIGHTY-EIGHT DOLLARS ($692,588.00) YEAR TWO (2) GRANT AWARD, NUNC PRO TUNC, TO SEPTEMBER 1, 2019, FROM THE CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC), RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH (REACH) GRANT; AUTHORIZING THE CITY MANAGER TO EXECUTE ANY AND ALL DOCUMENTS RELATED TO THE ACCEPTANCE OF THIS AWARD; PROVIDING FOR THE ADOPTION OF REPRESENTATION; PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, in an effort to support the work of Live Healthy Miami Gardens (LHMG), City staff submitted a multi-year grant to the Department of Human Services, Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community Health (REACH) (CDC-RFA-DP18-1813), and WHEREAS, year one (1) of this five-year grant was approved by City Council in January 2018, and WHEREAS, year two (2) of the grant award has been extend to the City, September 2019 through September 2020, WHEREAS, this 5-year CDC program is designed to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease, and WHEREAS, the REACH project will align with the practices promoted by the CDC's National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) program, and WHEREAS, the project's goal is to reduce chronic disease cases and deaths in Miami Gardens by increasing resident access of preventive and primary care resources, increasing the supply of and demand for healthy local foods, and transforming built and social environments to sustain improved physical activity levels and nutrition quality, and WHEREAS, the CDC REACH project is a multi-disciplinary, cross-sector collaboration of community stakeholders and organizations, and WHEREAS, as a requirement of the CDC's Notice of Funding Availability (NOFA), the initiative required to use a Collective Impact approach and partner with multiple organizations who would work collaboratively to implement the funded strategies. WHEREAS, the City of Miami Gardens has been awarded Six Hundred Ninety- Two Thousand Five Hundred and Eighty-Eight Dollars ($692,588.00) from the CDC REACH project, and WHEREAS, Staff recommends that the City Council accept the grant award for Year 2 from the CDC REACH, and allow the City Manager to execute any and all documents related to the acceptance of this award, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA AS FOLLOWS: Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas paragraphs are hereby ratified and confirmed as being true, and the same are hereby made a specific part of this Resolution. Section 2: ACCEPTANCE OF GRANT/AUTHORIZATION: The City Council of the City of Miami Gardens hereby accepts the Six Hundred Ninety-Two Thousand Five Hundred and Eighty-Eight Dollars ($692,588.00) award, nunc pro tunc, to September 1, 2019 from the Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community Health (REACH) grant. The City Council further Page 2 of 3 Resolution No 2019-264-3393 authorizes the City Manager to execute any and all documents related to the acceptance of this award. Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon its final passage.
