HomeMy WebLinkAbout2020-052-3450 Peacemaker Family Center Clinical Linkages Services RESOLUTION NO. 2020-052-3450 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA, ALLOCATING CENTER FOR DISEASE CONTROL AND PREVENTION (CDC) RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH (REACH) GRANT FUNDS; AUTHORIZING THE CITY MANAGER AND THE CITY CLERK TO EXECUTE AND ATTEST RESPECTIVELY THAT CERTAIN AGREEMENT WITH PEACEMAKER FAMILY CENTER (TRINITY CHURCH) FOR CLINICAL LINKAGES SERVICES; PROVIDING FOR INSTRUCTIONS TO THE CITY CLERK; PROVIDING FOR THE ADOPTION OF REPRESENTATIONS; PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, Live Healthy Miami Gardens (LHMG) and several community organizations submitted a multi-year grant application to the Department of Human Services, Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approaches to Community Health (REACH) (CDC-RFA-DP18-18130) resulting in the award of a five-year (2018-2023) grant to provide programming that improves health, prevents chronic diseases, and reduces health disparities among racial and ethnic populations in Miami Gardens with the highest risk, or burden, of chronic disease, and WHEREAS, on February 20, 2020, Request for Proposals, CDC REACH, was solicited via Miami Times and closed on March 5, 2020 and one (1) bid was received from Trinity Church/Family Peace Center and publicly read, and WHEREAS, on March 18, 2020, the Evaluation Committee met and reviewed the scope of services and the Trinity Church/Family Peace Center proposal was reviewed and firm was deemed responsible and responsive to the requirements of the Request for Proposal, and WHEREAS, it is in the best interest of the City of Miami Gardens to enter into an agreement with Trinity Church/Family Peace Center to provide clinical linkages services, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MIAMI GARDENS, FLORIDA AS FOLLOWS: Section 1: ADOPTION OF REPRESENTATIONS: The foregoing Whereas paragraphs are hereby ratified and confirmed as being true, and the same are hereby made a specific part of this Resolution. Section 2: AUTHORIZATION: The City Council of the City of Miami Gardens hereby allocates Center For Disease Control and Prevention (CDC) Racial And Ethnic Approaches to Community Health (REACH) grant funds and authorizes the City Manager and the City Clerk To Execute and Attest Respectively That Certain Agreement With Peacemaker Family Center (Trinity Church) for clinical linkages services. Section 3: EFFECTIVE DATE: This Resolution shall take effect immediately upon its final passage.
